Qualifications or Performance：1) The bidder shall have the independent legal person qualification, and register into a member at fslbxg.com. Domestic bidder’s bid documents shall provide copy of business license with bidder’s official seal. 2) Copy of bidder’s business license or business registration certificate (The business scope must include the bidding products, except bidders from abroad). 3) Have manufacturing liscence of medical equipment enterprise or business enterprises liscence of medical equipment for the bidding products (The business scope must include the bidding products, provide copy with official seal, except bidders from abroad). 4) Registration Certificate and Registration Form of medical equipment issued by National Food and Drug Administration (CFDA), and also Registration Certificate and Registration Form of imported medical equipment if it is imported equipment. (provide copy and stamped with the official seal) 5) The bidder must be a manufacturer or legal agent or the manufacturer’s directly authorised agent of this project (Provide copy of power of attorney with official seal). 6) Joint Venture will not be acceptable and subcontract will not be allowed. 7) Other qualification documents required in the technical documents for bidding.
